Records Requests

All record requests for the Nassau County Department of Health must be submitted in writing. To review records in person, please make a request in writing and provide a daytime phone number where you can be reached to arrange an appointment once the records are available. Please state the address for which you’re seeking records and describe the type of records you’re seeking. You can expect to receive an acknowledgment of your request within 5 business days, and most requests will be fulfilled within 20 additional business days. To expedite this process, the Department has several forms for certain programs that may be of interest for you.

Environmental Health Records Request Form

Realty Subdivision and Commercial On-Site Sewage Disposal Records Request Form
For records regarding the name of a subdivision map, a subdivision’s location (Village/City), and section/block/lot(s) of interest, and/or for records regarding the name of the commercial establishment, previous name(s), and address of a commercial on-site sanitary sewage disposal system please use this form here.

Please Note: In addition to the SWAP Records Request Form and as per the Nassau County Department of Health “Protocol for Review of Requests for Long Island Source Water Assessment Reports - September 3, 2003” the applicant is REQUIRED to indicate on his/her company letterhead, the reason for the request and how the information will be used. If approved, the Nassau County Department of Health will request that the applicant sign an agreement that the information obtained will not be distributed to other parties.

Medical Records 
For a copy of your or your child’s Health Department medical records, privacy laws require that you submit a signed HIPAA-compliant "Authorization Form to Release Health Information" available here.

Food Service Establishment Inspection Reports

Fees
There is a 25-cent, per-page fee for copying most records and copies of medical records cost 75 cents per-page. The fees for paper copies of Source Water Assessment Program records are 25 cents per-page and 50 cents per color map (8.5”x11”). The fees for duplicating engineering plans vary from plan to plan, therefore you will be notified of the cost for the specific plans you request.

You also have the right to review the records, by appointment, and can thereafter request copies be made for you. Depending on the volume of copies you request, copies may be mailed to you at a later date if they cannot be provided to you at that time.

Once the records are ready, the Health Department will inform you of the cost, and payment must be received by Records Access Officer before copies will be sent. Please include a copy of the letter the Department sent you acknowledging your record request with your check so we can track your payment and locate the records you’ve requested.
