Early Intervention (EI) Program (516) 227-8661

Children under three years of age with developmental delays or disabilities may be entitled to various EI services. Eligible children receive such services as speech therapy, physical therapy, occupational therapy, special education, family counseling, transportation and service coordination.

"Child Find" (516) 227-8661

This component of EI identifies infants at risk for physical and developmental disabilities. The program assures that they have routine developmental surveillance and screening, and provides referral for appropriate services.

Women, Infants and Children (WIC) Program (516) 227-9442

WIC provides food vouchers and nutrition education for pregnant women, infants and children. WIC also helps its clients access other health and human service programs.

Community Health Worker Program (516) 572-0934

This program serves at risk pregnant women and their families in the Village of Hempstead. Health workers conduct outreach in the community, make home visits, and provide education, advocacy and coordination of services to women.

Perinatal Services Network (516) 227-9456 

Information and referral service for pregnant and parenting women of Nassau County. The Network links women and their families to essential health and human services such as Prenatal Care and Day Care, regardless of citizenship or lack of insurance.

PHCP provides financial assistance to families of children from birth to age 21 who meet medical and financial eligibility.
